Prerequisites

A content type with `ezdate`, `ezdatetime` and `eztime` fields.

Steps

1. start creating content item of given type
2. fill all the fields

   First issue

   fields values have hardcoded format `mm/dd/yy, hh:mm AM`

3. click publish

   Second issue

   fields values in preview have the same hardcoded format

   Third issue

   content type names based on date fields also don't have the proper format

Expected result

Field values are formatted with long date time format
