Cannot create form in item with user field.

Steps:

1. Create a CT with fields: ezstring, ezform, ezuser (none required).
2. Create new item using this CT.
3. Click on Create a form button.

Actual: "Something went wrong" line is displayed instead of form builder, error occurs in browser console (screen attached)

TypeError: Cannot read property 'querySelector' of null
